While providing a home like atmosphere based on Godly principles, Endeavor will assist individuals with developmental disabilities to grow in the areas of personal care, daily living skills, emotional well being and spiritual understanding.
At Shepherds we encourage people to reach their maximum level of Appropriate Independence. Our Endeavor program accomplishes this by providing supports in each individual's daily life.
Each individual who lives in Endeavor has a Case Manager who works closely with them to develop and enhance skills. The Case Manager coordinates all aspects of the individual's life which include spiritual, medical, financial and vocational. Annual assessments are completed and goals are written which encourage growth and development. There is no “one size fits all” and goals are tailored specifically for each individual. These goals can be as simple as learning to brush teeth or as complex as balancing a checkbook. The Case Manager facilitates the annual Resident Assessment Meeting where residential, nursing and vocational staff meet with the individual to review and set goals for the upcoming year. The Case Manager then monitors these goals monthly and updates them as needed. Behavioral support is also provided by the individual's Case Manager.
Endeavor consists of 7 living units each housing 6 to 13 individuals. We also have 2 cottages that provide housing to 45 men. Each living area has a dining room, living room, bedrooms, and access to laundry facilities. The men and women generally find some of their closest friends in these smaller environments. They begin their day together, eat meals together, have devotions together, and relax in the evening as their day comes to a close. Each individual is encouraged to maximize their potential. Therefore, our supervisors and direct care staff spend time with each individual to help them develop and strengthen their daily living skills. These skills could include meal preparation, bathing, personal hygiene, dressing/undressing, laundry, cleaning, phone skills and safety.
Nursing is another service that is provided to those who live in our Endeavor program. Qualified nursing staff ensure that the individuals' medical needs are met. They also provide training for direct care staff in order for them to be well prepared to pass medications, understand medical needs and demonstrate good medical judgment.
The overall goal of our Endeavor program is to support the men and women who live here to grow in all aspects of their lives.
Endeavor Admission Criteria
Physical Requirements
-At least eighteen years of age
-Ambulatory
-Continent
-Able to feed self with supervision
-Free from communicable disease
-Free from multiple handicaps requiring special appliances or therapy
-Minimal medical needs
-Able to meet minimum Program Requirements
Mental Requirements
-Not emotionally disturbed beyond his/her ability to function within the framework of Shepherds' program.
-No maladaptative behaviors which might endanger him/her, other individuals or staff.
-An acceptable written action plan or medication must control minor behavior problems.
Financial Requirements
-One of the steps in the Admissions process is to meet with the Business Department relative to financial expectations.
-You may call the Business Administrator in advance to determine what materials might
be helpful at this meeting.
General Requirements
-An admission application and other current admission documents must be completed
on file.
-There must be at least two weeks of pre-admission respite evaluation at Shepherds
before final admission.
-Withdrawal/dismissal may occur by decision of parent/guardian or Shepherds.
-There must be a court appointed guardian or equivalent.
